For our public liability insurance no dog may enter our club grounds without us having sighted vaccination records showing a future expiry date.

Please use the form below to provide your dog's vaccination records ahead of course start date.

Make sure that both the date given and next due date shows clearly (not to be confused with batch expiry date of the vaccine given). Some vets are remiss at recording the date next due. If we can see the date given and the dog is aged 5 months plus we will assume a due date of 12 months from date vaccine was last given. Else, an email from your vet providing these details will be required.
